1929425 Primary Facility 10/22/2010 Routine/Complete Inspection No Violations Noted On October 22nd, 2010 I visited this site as a routine inspection. Drilling was completed but the well had not yet been fraced. The access road was in good condition. A Site ID and E&S plan were posted on site. E&S controls were constructed according to the plan. The pit was still in place and looked to be in good condition. Water trucks were on site removing fluid from the pit. There was no evidence of spills or leaks and no accelerated erosion was observed at the time of inspection.
1929504 Primary Facility 10/27/2010 Routine/Complete Inspection No Violations Noted On October 27th, 2010 I visited this site as a routine inspection. Crews were on site preparing for the frac. The access road was in good condition. A street sweeper was on site to keep the public roadway clean. A small portion of silt fence was becoming overtaken and the operator had a cew dispatched tor epair it while we were on site. A Site ID and E&S plan were posted on site. E&S controls were constructed according to the plan. The pit was still in place and looked to be in good condition. There was no evidence of spills or leaks at the time of inspection.

2186674 Primary Facility 07/16/2013 Complaint Inspection No Violations Noted I COULD NOT FIND ANY INDICATION OF LEAKS.INSPECTION FOR COMPLAINT #298246., TO SEE IF IT WAS POSSIBLE OIL SPOTS ON ANOTHER PROPERTY COULD BE FROM THIS WELL. THERE WERE NO OIL SPOTS ON THE PROPERTY JUST A WET SPOT DUE TO IN MY OPENION ABOVE AVERAGE RAINFALL. THE WELL HAS BEEN MAINTAINED AND IS IN GOOD CONDITION. THERE IS A DUMP SYSTEM AND BRINE TANK.
