2370620 Primary Facility 05/14/2015 Routine/Complete Inspection Pending Well appeared to be equipped for production, brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that immediately down slop from the well the landowner had recently plowed the field. There was no well tag observed on any of the well apparatus. At this time the Department is requesting that the operator contact WQS Higgins at the belwo listed phone number or forward documentation to the above listed address no later than May 27, 2015 that a well tag with the appropriate permit number has been affixed to the well apparatus.
