The well location is/was located in a cultivated crop field. There was no monument bserved onsite and it was inferred that the monument is buried. At this time, due to the lack of documents in the Department's hard copy file the Department is requesting that the operator forward the following documentation: 1) A copy of the Notice of Intent to Plug that was last submitted for the permit. 2) A copy of the Certificate of Well Plugging.
