2006341 Primary Facility 09/13/2011 Routine/Complete Inspection No Violations Noted The intent of the inspections was to assure the flowback water did not exceed the 2¿ freeboard requirement as per 25 PA code 78.56 (a) (2) and the liner did not experience compromised integrity as per 25 PA code 78.56 (a) (4), due to recent rainfall events from tropical storms. During the inspection, the freeboard level was observed to be right at the 2¿ freeboard level. Compromised integrity of the impoundment liners was not noted. Jason Petlock of Southwestern informed us that prior to 9/14 the freeboard level was slightly exceeded in the 40,000 barrel capacity flowback water only impoundment; Permit number ESX10-015-0078. Southwestern transferred water from this impoundment to vertical holding tanks in the tank farm area to keep the surface water elevation as low as possible. Some staining and water marks were observed form the surface of the water to the top portion of the liner. The 200,000 barrel capacity flowback/fresh water blended impoundment, Permit Number ESX10-015-0072, did not experience freeboard exceeding and staining.

2074338 Primary Facility 06/08/2012 Incident- Response to Accident or Event Adverse Events or Actions Reported Inspection part of a contunuing investigation into the identified chloride elevated seeps located on the fill slope of the tank farm pad. Sample # 0257070 was collected from the discharge of the small laggon underdrain discharge. Primary focus of investigation is to determine the status of the integrity of the small lagoon liner. Large lagoon liner integrity is not in question at this time.

2099253 Primary Facility 09/10/2012 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Inspection conducted on 9/10/12 with Rich Gulyas. 2 new monitoring wells have been installed. One is located below the small impoundment, and the other is located below well pad and between the large and small impoundment. Drill cuttings and casing sealant debris is located around both new monitoring wells. Rich said he would get rid of this debris ASAP. The sump pump located in the sump cellar which was installed in the rocked swale right above the small impoundment is not functional. Rich not sure why. Please send an email explaining your intentions for this sump pump.

2603354 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted On June 6, 2017, at 1200 hrs., the Department conducted a routine inspection at the SWN Production Company LLC, GU-C-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A SWN representative was on site performing maintenance work on the adjacent waste impoundments.

The site sign, E & S plan and the weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. PCSM has not been addressed at the site, but the site has been re-permitted and upgraded to an ESCGP-2. PCSM installation is scheduled for the month of June.

Seven wells were in production phase during the inspection. All of the wells were labeled and no bubbling was observed in the well cellars. Two production tanks and seven GPUs were also observed on site. The production tanks were observed in secondary containment. Puddles on the pad surface were low in specific conductivity. A small area of stained gravel was

2818307 Primary Facility 2018-12-18 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Greenzweig (C Pad) in Herrick Twp, Bradford County on 12/18/2018 at 1155 hours. The inspection was conducted in response to a Well Site Restoration Report received by the DEP.

The site has recently been disturbed to install PCSM BMPs per restoration plans. The areas that were disturbed now have straw/hay layered to assist in vegetation growth in the Spring. The areas are poorly vegetated now and are not stabilized. Silt sock was observed beneath the newly constructed Bioretention basin. This site does not meet the requirements for restoration under a well site restoration report. The site is not stabilized with 70% vegetative cover and temporary E&S BMPs are still installed. It is understood that Southwestern has a Consent & Order agreement with the department to install these newly permitted PCSM BMPs. Upon further discussion, the sites already inspected will have to be returned.

This Well Site Restoration Report is considered returned at this time.
