2589066 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted On 4/20/2017, the Department performed an inspection of the Rice Energy Glass Joe well pad site. Present at time of inspection was Clay Clifford (Rice). The site is located in Aleppo Twp., Greene Co. The inspection was a routine inspection during setting of conductors for newly permitted wells on the site.

The operator has removed a majority of the E&S BMPs. The PCSM structures have been installed. The site is stable with greater than 70% vegetative cover.

The operator has set 5 well conductors on the site in preparation for future drilling on these wells. Conductors have been set to 40' on the 1H, 2H, 4H, 6H, and 3H.

2846258 Primary Facility 2019-02-28 Routine/Complete Inspection No Violations Noted The well has been partially plugged. The horizontal portion of the well has been abandoned. The vertical portion has been cemented to ~1,592'. The well appears to be tagged with the incorrect permit number.
2848325 Primary Facility 2019-03-05 Routine/Complete Inspection No Violations Noted The well has been partially plugged. Prior to resumption of plugging, the operator is on site measuring casing annulus pressures. 72 psi observed on 9 5/8" x 13 3/8" annulus gauge. Annulus blew down quickly. 5 1/2" x 9 5/8" annulus was unable to be measured.
