2627709 Primary Facility 2017-06-13 Incident- Response to Accident or Event No Violations Noted On 06/12/17 at 1143 hours Dennis Furmanek of Rice Energy reported to the Department a spill of 50 gallons of diesel fuel had occurred at approximately 0945 hours at the site. Furmanek reported SPSI would be performing the cleanup and that absorbent materials were placed immediately after the spill.

On 06/13/17 at approximately 1300 hours an incident response inspection was conducted. The Act 9 sign was in place and all required documents were present at the site. The spill area was being excavated to remove impacted material at the time of inspection. The spill has resulted in a violation of SWMA 301.

No water shows identified during drilling of surface and coal borehole sections. Conduct site walk. Site in good condition. All depths from KB elevation. KB = 12.5' Operator nippling up on 13 3/8" coal protective casing.

2667579 Primary Facility 2017-11-28 Compliance Evaluation No Violations Noted The Act 9 sign was in place and all required documents were present at the site. A spill of approximately 50 gallons of diesel fuel occurred on 06/12/2017 resulting in an NOV being issued for SWMA 301. The spill area had excavated and remediated. There were no violations observed at the time of inspection.

2826251 Primary Facility 2018-12-17 Routine/Complete Inspection Violation(s) Noted On 12/17/18 at approximately 1130 hours I met with David Guidas of EQT in response to a spill that occurred on 12/14/18 (inspection ID # 2826184). During the inspection multiple GPUs being operated without secondary containment were observed.

API 37-059-27280 was SPUD 08/14/17. The well is serviced by GPU 596636, which did not have secondary containment. A violation of 78a64(a)b occurred.
