2597416 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted On May 24, 2017, at 1045 hrs., theDepartment conducted a routine inspection at the Chesapeake Appalachia, LLC, Wygrala Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated and no erosion issues were observed. An infiltration basin was installed at the site and was functioning properly. Cuz Excavating was on site working to install an infiltration berm for the pipeline valve site near the well pad.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. Two production tanks and one GPU were also on site. Puddles on the pad surface were low in specific conductivity. No staining was observed on the well pad surface.

The well pad has not been reduced in size, but PCSM has been addressed at the site.

2837942 Primary Facility 2019-02-08 Site Restoration No Violations Noted I took electrical conductivity readings with an Oakton PCSTestr 35. A puddle near the production tanks is 0.12 mS/cm.
