2632586 Primary Facility 2017-08-02 Routine/Complete Inspection No Violations Noted On August 2, 2017, the Department performed an inspection of the EQT State Gamelands 179 well pad site. Present at time of inspection was Bill Balog (EQT), Will Brogan and Mike Stephan (DEP). The site is located in Aleppo and Jackson Twps., Greene Co. The inspection was a follow up inspection of the site after noted violations on July 31, 2017.

The operator was in process of repairing E&S BMPs which were damaged and failed during a recent heavy rainfall event. The operator has agreed to repair the stream crossing for Reinhart Rd. In Jackson Twp. The operator will apply for necessary Emergency 105 permit to perform repairs to the stream crossing and to remediate impacts to an unnamed trib. to Job Creek from sediment discharges associated with E&S BMPs failure on site after the recent rainfall event. The remediation will be performed by hand as much as possible.

13 wells have been permitted for the site. Site under construction. No wells have been drilled at time of inspection.

2636826 Primary Facility 2017-09-07 Routine/Complete Inspection No Violations Noted On September 7, 2017, the Department performed an inspection of the EQT State Gamelands 179 well pad site. Presnt at ime of inspection was Rex Nelson (EQT). The site is located in Aleppo and Jackson Twps., Greene Co. The inspection was a routine inspection after notification by the operator of E&S failures on site after a recent rainfall event.

The operator has repaired E&S BMPs which were damaged and failed during a heavy rainfall event. The operator reported a rainfall of 1 1/2" during a 1 hour timeframe on September 5, 2017. During the September 6, 2017 E&S inspection the operator noted failures along the silt sock down gradient from the excess material stockpiles. The operator has stabilized the site with armor guard hydro seed as areas of construction are complete. The erosion prevention seeding did not perform as expected and sediment laden water over toped the silt sock discharging small amount of sediment approximately 20' outside the LOD. 2651368 Primary Facility 2017-10-05 Preoperation Inspection No Violations Noted On October 5, 2017, the Department performed an inspection of the EQT State Gamelands 179 well pad site. Presnt at ime of inspection was Rex Nelson (EQT). The site is located in Aleppo and Jackson Twps., Greene Co. The inspection was a inspection after a request by the operator for a pre-operational inspection.

The operator has completed installation of all E&S controls. The well pad final grading and stoning is being completed. The operator has completed construction of the access road and PCSMs along the access road. The construction of the well pad PCSMs (wet pond) is almost complete. The operator has temporarily stabilized the well pad and stockpile slopes with hydro seeding and slope armoring.

13 wells have been permitted to be drilled on site. None of the wells have been drilled at time of inspection.
