2650489 Primary Facility 2017-10-03 Site Restoration No Violations Noted On October 3, 2017, the Department performed an inspection of the Rice Energy Walter Stinger well pad site. Present at time of inspection was John Carson (DEP). The well pad is located in Aleppo Twp., Greene Co. The inspection was related to request by the operator to terminate the ESCGP2 permit.

The Department entered into a COA with Chesapeake Appalachia LLC, dated 11/8/2015. The COA required CALLC to remediate the impacted, unnamed trib., to Harts Run, by the initial slope failure, and complete stream enhancements along Harts Run and an unnamed trib., to Mudlick Fork. The COA also accepted the stabilized and reconstructed north well pad slope as-built as approved ESCGP2 permit. The site has been transferred to Rice Energy and Rice has requested to terminate the ESCGP2 permit. The site is stable with greater than 70% vegetative cover, but triple stack silt sock and three sediment traps remain inplace (E&S BMPs). The operator will need to remove the silt sock and reclaim the sediment traps prior to ter

2680040 Primary Facility 2018-01-09 Drilling/Alteration No Violations Noted Well permit on site. The well was spud on 12/28/17. The operator drilled a 30-inch borehole to 40 feet and set and cemented a 26" x 40' conductor pipe. Cellartech well cellar in place.

Operator installing containment. Rig moving on site.

2680204 Primary Facility 2018-01-04 Routine/Complete Inspection No Violations Noted On January 4, 2018, the Department performed an inspection of the EQT Walter Stinger well pad site. The site is located in Aleppo Twp., Greene Co. The inspection was a routine inspection after restoration of the site and conductor set for two new wells for the site.

The access road and well pad are in good condition.

The operator has removed all E&S BMPs to include the three sediment traps which were on site. The operator has set conductor on two newly permitted wells for the site.

The 8H well is still listed as inactive, and operator has set conductor on the 10H and 12H wells.

2684248 Primary Facility 2018-01-19 Drilling/Alteration No Violations Noted Surface and coal protective casings have been set and cemented to the surface. Operator has drilled intermediate borehole section to 2,736' (KB) and installed intermediate casing. Operator cementing 9 5/8" intermediate casing.

Pittsburgh Coal = 1,307' to 1,315' (KB) KB = 12'

2688396 Primary Facility 2018-01-30 Drilling/Alteration No Violations Noted The operator has drilled the tophole portion of the well. Surface, coal, and intermediate casings have been set and cemented. The operator has drilled the pilot hole to 6,372' MD. The well is capped. Operator is currently drilling well 059-27553. Site is snow covered at the time of the inspection.

All depths from KB elevation KB - 12.5'

2758120 Primary Facility 2018-07-31 Drilling/Alteration No Violations Noted Operator has completed drilling to a total depth of 18,453'. Operator is running production casing in hole @ 8,633' at the time of the inspection.
2770593 Primary Facility 2018-08-29 Routine/Complete Inspection No Violations Noted On August 29, 2018, the Department performed an inspection of the EQT/Rice Energy Walter Stinger well site. Present at time of inspection was Andrew Harsh and Aaron Parker (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was routine inspection during hydraulic fracturing operations, related to a request by the operator for a minor modification to the ESCGP1 permit.

The operator has requested a minor modification to the ESCGP1 permit to add rock check dams in the access road collection channel as needed. The minor modification was reviewed and approved by the Department in the field.

At the tome of the inspection, the well pad was initially shutdown, due to a 10 gallon biocide spill on the secondary containment. The well pad was cleared to resume hydraulic fracturing operations. The AST on the well pad was filled with freshwater to test the containment. During hydraulic fracturing operations the tank will hold fresh and reuse water. The well pad secondary containment was in good co

2777740 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted On September 12, 2018, the Department performed an inspection of the EQT/Rice Energy Stinger well site. Present at time of inspection was Carey Bell (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was routine inspection during hydraulic fracturing operations.

The operator has requested a meeting on site to review the condition of the site prior to submitting a Notice of Termination for the ESCGP permit.

The well site is stable with greater than 70% uniform perennial vegetative cover. The operator has reclaimed the three sediment traps on site and has removed all the E&S BMPs except for the triple stacked silt sock along the bottom of the north facing well pad fill slope. The Department would recommend termination of the ESCGP permit after the operator removes the last of the E&S BMPs on the fill slope, given the condition of the site at time of inspection.

The AST on the well pad was filled with reuse water and appeared in good condition. During hydraulic fracturing operations t

2788825 Primary Facility 2018-10-10 Routine/Complete Inspection No Violations Noted On October 10, 2018, the Department performed an inspection of the EQT/Rice Energy Stinger well site. Present at time of inspection was Carey Bell and Harry Good (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was a restoration inspection related to a Notice of Termination for the ESCGP1 permit by the operator.

The operator has submitted a Notice of Termination for the ESCGP1 permit.

The well site is stable with greater than 70% uniform perennial vegetative cover. The operator has reclaimed the three sediment traps on site and has removed all the E&S BMPs. Given the condition of the site during the inspection, the Department recommends termination of the ESCGP1 permit.

The AST on the well pad was filled with reuse water and appeared in good condition. During hydraulic fracturing operations the tank will hold fresh and reuse water. The well pad secondary containment was in good condition and functional.

The operator will need to remediate the small hydraulic spill on the well p
