2565968 Primary Facility 2017-03-01 Routine/Complete Inspection No Violations Noted On February 28, 2017, at 1100 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Lillie Well Pad location. Nobody was present for Chesapeake. Andrew Hobbins, Water Quality Specialist, represented the Department.

The E&S plan, site sign and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. PCSM has been addressed for the Lillie location. No significant erosion issues were observed

There were two wells in production phase and labeled during the inspection. The well cellars were for both wells were filled with water, but no bubbling was observed. Two GPU's and two production tanks were also on site. The production tanks were in secondary containment. Two shipping containers were stored on the southwest corner of the well pad. There was no indication as to the contents of the shipping containers. There was no staining observed on the well pad surface.

2636127 Primary Facility 2017-09-11 Site Restoration No Violations Noted On September 11, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia, LLC, Lillie Location in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake. The inspection is in response to a Notice of Termination (NOT) submittal for the Lillie Location ESCGP.

Generally the well pad was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. The slopes of the pad and topsoil stock pile were observed to have greater than 70% perennial vegetation. Three infiltration berms were observed to be installed as per the approved ESCGP. The well pad has not been reduced in size, but PCSM has been addressed.

No violations were observed. Based on condition observed during the inspection, the Department recommends approving the NOT.
