2576555 Primary Facility 2017-03-06 Routine/Complete Inspection No Violations Noted On March 6, 2017, at 1310 hrs., the Department conducted a routine inspection at the SWN Production Co. (SWN) GU-T-Pad in Herrick Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for SWN.

The E&S plan, site sign and weekly inspection logs were on site and available for review. The slopes of the well pad had good vegetative growth. The Department did not observe any erosion issues.

There were six wells in production phase during the inspection. All wells were labeled and no bubbling was observed in the well cellars. There were two production tanks and six GPU's on the well pad. No staining was observed on the well pad surface.

The ESCGP-2 expires on January 16, 2022. According to Department records, there are no active well permit for the GU-T-Pad. The Department recommends installing the PCSM BMPs that are proposed in the ESCGP-2.

2613207 Primary Facility 2017-06-28 Routine/Complete Inspection No Violations Noted On June 28, 2017, at 1050 hrs., the Department conducted a routine inspection at the SWN Production Company, LLC, GU-T-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Representatives from Meshoppen Stone were on site installing PCSM BMPs that were proposed by SWN as a result of re-permitting the site and upgrading to an ESCGP-2. The ESCGP-2 was approved by the Department on January 17, 2017.

The Department observed Meshoppen Stone workers constructing the berm by removing topsoil from the on site topsoil stockpile. An underdrain system will be installed as part of the bioretention berm using 4" perforated PVC which will outlet to a rip-rap apron. The Department would like to remind SWN that, according to the bioretention berm maintenance schedule, "the underdrain gate valve should remain closed at all times, except when water is observed in the bioretention berms 72 hours after a storm event." 2653515 Primary Facility 2017-10-23 Site Restoration No Violations Noted On October 23, 2017, the Department conducted a routine inspection at the SWN Production Company, LLC, GU-T-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Generally the site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. All compost filter sock has been removed from the site. The well pad has not been reduced in size, but PCSM has been addressed.

Based on the conditions observed during the inspection, the Department recommends the Notice of Termination be approved.
