2780914 Primary Facility 2018-09-25 Routine/Complete Inspection No Violations Noted The DEP conducted an inspection of the Repsol Oil & Gas Stephani (03-111) K well pad in Columbia Twp, Bradford County on 9/25/2018 at 1040 hours. The weather was rain, 59 degrees, and 90% humidity.

The inspection was conducted in response to a well site restoration report received by the DEP for API #015-21907. All PCSM BMPs appear to be in place and functioning properly. No temporary E&S BMPs were observed and the site appears to be stabilized. No erosional issues were noted.

This Well-Site Restoration Report is considered accepted by the DEP. Due to all site restoration requirements being met, I would also recommend this site for termination of its ESCGP upon submission.

No violations noted. I departed the site at 1110.
