2660023 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted I met Jeremy Boyd when I arrived. Precision rig 569 on location, just finishing drilling the 12 1/4" section of the Hart North 5H. Containment was in fairly good shape, although I pointed out 2 areas to Jeremy where the liner was torn on the berms. Jeremy told me he would address it. Overall, site appeared to be clean and well maintained.

Universal conducting the frack. PPC site specific plan on site. The operator reported approximately 200 gallons of fracking fluid escaped at high pressure from an iron fitting attached to the well head. The high pressure fluid cut through the primary containment. The iron fitting was replaced and the containment was repaired. No clean-up under containment. A Vac truck was used were free liquid could be removed. Spilled material appears to have stayed on pad surface. Clean-up to begin after the frack is completed. Violations noted. No samples collected.

2707081 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted Site was very active when I arrived; company man Gibb Blevins was not at the site when I arrived. Frac was finished, and crews were setting up temporary separators and dehy units, as well as breaking down Baker tank containments. Many puddles were present on the pad surface, with conductance ranging from .56 to 1.3 mS/cm for the most part. Some small puddles (.5-2 gallons) were present near one of the Baker tank batteries that had slightly higher conductance (2-2.5 mS/cm); other puddles in the immediate area were in the range of .8-1.1 mS/cm. I was unable to access the center of the pad due to activity and truck traffic.
2733696 Primary Facility 2018-05-22 Routine/Complete Inspection No Violations Noted Arrrived at 12:00. Weather: rain. There were 5 producing wells on location. Frac operations appear to have been recently completed. There were 5 sand separators and two banks of tanks on site, liner was still in place. Cellars were water-filled with the exception of the Hart N 3H. It is worth remarking that there were numerous signs of ineffective sediment control on the access road (see photos).
