2700515 Primary Facility 2018-02-22 Routine/Partial Inspection No Violations Noted At the time of inspection, well was producing. Well integrity was in good condition with no signs of any leaks or spills. Well is labeled and has a cage fence around the well. Annuli are open to tanks. There are 4 tanks (3 are for produced water and 1 for condensate). The tanks are within a lined, steel dike. There were also 3 line heaters within a second lined, steel dike. There was also a gas buster hooked up to the tanks to handle any build up of gas within the tanks. Production is being reported to the Department.
2715591 Primary Facility 2018-03-27 Routine/Partial Inspection No Violations Noted At the time of inspection, well was producing. A request was received to inspect pad because there had been "white smoke" coming from the pad. XTO had already determined that it was an overheating line heater that was creating steam. During my inspection, there was a small amount of steam, but was only due to rain hitting the hot elements.
2727944 Primary Facility 2018-05-09 Routine/Partial Inspection No Violations Noted On 5/9/18, WQS Brian MacQuarrie onsite. Site sign installed and maintained at entrance. Three wells in production; the 1H-019-22575, 2H-019-22576 and 3H-019-22577. Erosion and Sedimentation best management practices installed and maintained. Vegetative growth established in previously disturbed areas.
2731881 Primary Facility 2018-05-21 Routine/Partial Inspection No Violations Noted On 5/21/2018, WQS Brian MacQuarrie onsite. Site sign installed and maintained at entrance. Access road maintained. Three wells in production; the 1H-019-22575, 2H-019-22576 and 3H-019-22577. Erosion and Sedimentation best management practices installed and maintained including sediment trap, infiltration berm, rip-rap channels, silt fence and culver pipe inlet/outlets. Vegetative growth established in previously disturbed areas.
2792366 Primary Facility 2018-10-22 Site Restoration No Violations Noted On 10/22/2018, WQS Brian MacQuarrie onsite for the purpose of conducting a Notice of Termination/site restoration inspection. The site consists of three wells in production, 1H-019-22575, 2H-019-22576, and 3H-019-22577. Site has four 400bbl production tanks installed within containment and three gas processing units. Post Construction Stormwater Management BMPs installed and maintained. Site well vegetated in previously disturbed areas. Permanent stabilization achieved according to 25 Pa. Code 102.22(a)(2).

Therefore, the Notice of Termination request is approved.

2813108 Primary Facility 2018-12-06 Routine/Partial Inspection No Violations Noted On 12/6/2018, WQS Brian MacQuarrie onsite. Well site consists of three wells in production 1H-019-22575, 2H-019-22576 and 3H-019-22577. Tank battery consists of four 400 bbl tanks installed in secondary containment.
