2672428 Primary Facility 2017-12-15 Routine/Complete Inspection No Violations Noted Routine site inspection of Seneca Resources CRV PAD C08-X on December 15, 2017. There are a total of 7 producing Marcellus wells located on this well pad. Stationed near the well heads in secondary man-made containment is a frac tank with a berm. There are 7 gas production units located on the E edge of the well pad. In addition, there are (5) 400 BBL produced fluids tanks stationed in a modular secondary cobtainment along the N edge of the well pad. Water within this containment is frozen and snow covered.

The well pad and surrounding areas are snow covered. E&S controls and PCSM are in place and appear to be functioning as design.

A notice of termination has not been approved to date for this ESCGP and the well pad remains at full built out.

2692469 Primary Facility 2018-02-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The wellhead assembly, all flow lines, fittings, and vented annulus was tested for gas. Results of this testing was that gas was detected on the 9 5/8"x 5 1/2" vented annulus. No other gas was detected on the well or the well area.
2716807 Primary Facility 2018-04-10 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The annulus vents , all fittings and flow lines were tested for combustable gas. Results of this testing was that no combustable was detected on the wellhead assembly or the well area.

2765955 Primary Facility 2018-08-16 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources CRV Pad C08-X on August 16, 2018 with company man Justin Greene. The site sign, weekly E&S log, drilling permits, emergency response plan, and PPC plan were onsite for review.

There are currently 7 producing Marcellus wells located on the well pad. The operator is currently in the process of drilling 1 of 6 Utica wells. All six Uitca wells have been spud. The operator was is in the process of drilling Utica well 023-20235 using Patterson 328 Rig. The drilling depth was at approximately 6,650 ft. during the Departments inspection. The kick-off point is at 10,500 ft. with a total depth of 21,000 ft.

All rig equipment, tanks, silos, diesel fuel tanks, oils, etc. have been placed on the primary rig timber mat containment. An additional layer of secondary containment separates all of the equipment listed above. Overall, the containment system looks great, no water was observed laying on the containment. A dedicated v

2813550 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources CRV Pad C08-X on December 11, 2018. The site sign, weekly E&S log, drilling permits, emergency response plan, and PPC plan were onsite for review.

There are currently 7 producing Marcellus wells located on the well pad. The operator just finished drilling Utica well 023-2033 and then walked the Patterson 328 rig over to begin drilling on well 023-20230. Drilling on Utica well 033-20230 just began today (December 11, 2018).

All rig equipment, tanks, silos, diesel fuel tanks, oils, etc. have been placed on timber rig mats serving as primary containment. An additional layer of secondary containment separates all of the equipment listed above and consists of geotextile and additional liners. Overall, the containment system looks great, no water was observed laying on the containment. A dedicated vacuum set system is stationed onsite within secondary containment. Freshwater is being trucked onsite and offloaded into

2850643 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources CRV Pad C08-X on March 6, 2019.

There are a total of 7 producing Marcellus wells located on this well pad. Each well is currently enclosed in a yellow cage. In addition, there are 6 recently drilled Utica wells on this pad. Each Utica well head is currently wrapped in black fabric. Adjacent to the Utica wells are 5 frac tanks stationed in secondary containment with a berm. Snow was observed within the containment system,

There are 7 gas production units located on the E edge of the well pad. In addition, there are (5) 400 BBL produced fluids tanks stationed in a modular secondary cobtainment along the N edge of the well pad. Water within this containment is frozen and snow covered.
