2054016 Primary Facility 03/21/2012 Routine/Complete Inspection Violation(s) Noted At the time of this inspection, well drilled and in prodcution. Well tag present. Drilling completed 1/8/06. Site restored with adequate vegetation. No accelerated erosion or sedimentation issues noted. Department observed a "puddle" bottom of brine tank near valve. After closer examination department observed a "Steady slow leak" from brine tank. Department tried to call operator but recieved answering machine, no message left. At approximately 1330 depaartment was able to locate an emergency number for operator. Deaprtment left a message with answering service of brine release at said well. Aprroximately 1-1/12 later Christine Marie Rep returned my call and was promptly gonig to have the tank pumped out and adddress the issue. A follow up will be conducted in the am to ensure proper cleanup measures are executed.
2054024 Primary Facility 03/22/2012 Follow-up Inspection Outstanding Violations - No Viols Req'd Follow up inspection to brine release discovered by the department during a routine inspection. During this inspection, brine tank actively being pumped out. Valve was closed, it appears leak has been stopped. An undeterrmined amount of brine was released from the tank onto the ground. CM rep stated that valve was closed last night around 500pm. It does not appear that brine has accelerated offsite or cause any major erosion, operator has plans to remove "visibly" impacted/contaminated soil and replace with rock. Operator believes the release amount was minimal, at this time an exact amount could not be determined, well lasted inspected by operatort on Monday 3-20-12. A follow up will be conducted early next week to examine completed remediation measures. No new violations noted during this inspection, issued violations are outstanding until final remediation measures are completed.
2054579 Primary Facility 03/28/2012 Compliance Evaluation No Violations Noted CEI inspection to brine release discovered by the department during a routine inspection on 3/21/12 Well tag present, well equipped and in production. No accelerated erosion or sedimentation issues on well site or access road. Department received voicemail message on Friday March 23, from operator that site has been remediated. As a result department is conducting CEI inspection. During today’s inspection, it appears approximately 3-4inches of previously impacted soils have been removed and fine stone placed in excavated area. Again, it does not appear that release migrated offsite. Operator unable to quantify how much brine was released onto the grounds of commonwealth prior to tank being pumped.
