2494873 Primary Facility 2016-06-20 Routine/Complete Inspection In Compliance with Policy At time of inspection no E&S issues observed on gated access road. No E&S issues observed on site. Well appears equipped for production. Well tag present and legible. Brine tank appears structurally sound however Department observed area of no vegetation directly down-gradient of site that may be indicative of a brine release. Area of potentially impacted soil is approximately 300 feet SSW of site. Area is located in a low area at the toe of a slope and continues around toe of slope approximately 200 feet to the west. Area varies in width from approximately 30 feet to approximately 16 feet and it appears there are many points of exit, not one single source. Department attempted to contact Operator rep Chris Baker by phone from site. Department collected soil samples; 1 background and 2 of potentially impacted soils.

Inspection is PENDING receipt and review of soil sample results from Department lab. Violations may be forthcoming pending soil sample results. UPDATE 5-5-17: Department received and reviewed so

2494883 Primary Facility 2016-06-21 Administrative/File Review In Compliance with Policy Follow up inspection to PENDING inspection record #2494873 where Department collected soil samples to determine if a brine release has occurred. Operator Rep Chris Baker returned Department’s phone call from 6-20-16. Department met Mr. Baker at site and walked area of potential impact. Department collected additional soil samples; 2 background and 2 of potentially impacted soils.

UPDATE 6-22-16: Operator e-mailed Department and stated brine tank was last hauled on 7-28-15. There were 45 bbl of brine removed and 20 bbl left in tank. The brine is used for dust control via road spreading in Green Twp, Indiana Co. Also, the well does not produce much brine. Mr. Baker also stated that there was not an encapsulated pit on site. Operator is In Compliance with Policy at this time in as far as Operator has agreed to provide Department with information regarding this site. Department recommends Operator conduct their own investigation in conjunction with Department's. Violations may be forthcoming (see PENDING inspec
