2171159 Primary Facility 06/03/2013 Compliance Evaluation No Violations Noted A Compliance Evaluation Inspection was conducted at the Hoopes 14 well site in Ulysses Township, Potter County on June 3, 2013. During an inspection (2133388) conducted on January 23, 2013 multiple violations were cited. Today's inspection found the site to be restored to original contours, the drill pits has been encapsulated and backfilled and a well tag installed on the plugged well. The area has been temporarily stabilized with mulch. Once the site has been stabilized with a minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation a Well Restoration report in accordance with 25 PA Code § 78.65(3)(which states that within 60 days after restoration of the well site, the operator shall submit a well site restoration report). The violations cited on January 23, 2013 are considered resolved.
