2190171 Primary Facility 08/08/2013 Routine/Partial Inspection No Violations Noted On August 8, 2013 an inspection was conducted at the Ken-Ton 902 pad in West Branch Township, Potter County. The 1H, 2H, 3H, 4H, 5H and 6H wells are drilled and are in production. The 7V well is drilled, but not in production, the status of the well is Inactive. The 9H well is not drilled, but the permit is active. The well permits posted on site are destroyed / missing, the site ID sign is in need of repair and updating. The site is stable and well vegetated. The conductance of the puddles screened on the pad surface ranged from 200 - 850uS/cm. No issues were noted and no violations were cited during this inspection.
2317672 Primary Facility 10/28/2014 Routine/Complete Inspection No Violations Noted Routine inspection. 1H - 6H & 7V wells are drilled. Ultra Resources completed site restoration. Site to be transferred to SWEPI LP. TLT

2623195 Primary Facility 2017-08-07 Site Restoration No Violations Noted On 8/7/17 at 0902 hrs I conducted an inspection of the Ken Ton 902 well site. No one was present during the inspection. It was overcast and 61 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the access road and well pad. Seven wells, the Ken Ton 1H - 6H and 7V (Photos #1-3), have been drilled. According to DEP records, the 1H - 6H wells are in production. The 7V well was placed on inactive status on 1/29/13.

Produced water is stored in a production tank, which has built-in containment (Photo #4). Routine secondary containment inspections are being completed; the last inspection was on 8/5/17. Puddles on the pad surface were tested for field measured specific conductance; readings ranged between 531 - 1,030 micro Siemens/cm.

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed (Photo #4). Temporary E

2643547 Primary Facility 2017-09-12 Routine/Complete Inspection No Violations Noted Well design is a 13 3/8" surface casing, a 9 5/8" intermediate and a 5 1/2" production string. The 5 1/2" production string is vented to surface.
2684669 Primary Facility 2018-01-17 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was producing. The wellhead assembly all fittings and flow lines were tested for gas. The results of this testing was gas was detected on the 9 5/8" x 5 1/2" vented annulus. No other gas was detected on the wellhead assembly or the well area.
2751242 Primary Facility 2018-07-12 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP-Ken Ton 902 well site located in Elk Township, Tioga County on July 12, 2018 @1200 hrs. No one was present during the inspection. The weather was 74 degrees and sunny, humidity 38%, winds NNE 5mph.

I observed the access road and well pad. The site consists of seven wells, the Ken Ton 1H - 6H and 7V have been drilled. According to DEP records, the 1H - 6H wells are in production. The 7V well was placed on inactive status on 1/29/13.

Produced water is stored in a production tank, which has built-in containment. Routine tank inspections are being completed; the last inspection was on 6/12/18.

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. The NOT was approved on 8/7/2018.

No violations are noted at this time. I departed the site at 1315 hours.

2831244 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted At the time of this inspection this well in production. A digital gauge on the well head assembly was showing a pressure of 265 psi (pounds per square inch). The valve on the production/flow line was in the open position. A single annulus vent that is the intermediate x production was tested for combustible gas with the results being 100% LEL (lower explosion limit) and 28% METHNE. No other annulus vents are vented to surface at this well. No violations noted at this inspection.
