2833832 Primary Facility 2019-01-30 Incident- Response to Accident or Event Violation(s) Noted The DEP conducted an incident inspection of PGE's Pine Hill 1941 Well Pad in Wharton Twp, Potter County on 1/30/2019 at 1215 hours. The inspection was conducted in response to a spill reported to the DEP Report hotline by Nathan Harris (PGE). It was reported that at approximately 0900 a vac truck that was moving produced water from a production tank had "collapsed". The tank was estimated to have had 25 barrels of liquid inside when it collapsed.

DEP WQS, Jonathan Pennings and I arrived onsite at 1215 hours. When we arrived the truck was still in place and crew was observed sucking up fluids from the pad surface. We observed the vac truck(see photo #1) and the impacted area (photo #2 & 14). Puddles and slush mixes were field measured with a PCSTestr returning readings ranging from ~2.0 ms/cm to ~15.0 ms/cm (see photos #3, 4 & 5).

We spoke with Erick Duzick (PGE Completions Engineer) onsite who explained the vac trucks relief valve was frozen solid which had caused the truck to create a vacuum which in ret
