2017568 Primary Facility 11/08/2011 Routine/Complete Inspection Violation(s) Noted An inspection was conducted at the Fisk Hollow Pad B on 11/8/11. The site ID sign and E&S plan and site perimeter were observed. The access road was installed, stabilized and the accociated BMPs functioning. The perimeter BMPs were installed and functioning. The site were stablized. A release of a pollutional substance was discovered near the production tanks. A hook-up for pumping out the contained fluid was located off the containment and was leaking. A sample was collected and submitted for SAC 942 analysis. Violations were cited during this inspection for release of a pollutional substance (25 PA Code 78.57, SWMA 301 and 402 CSL). During the inspection, John Mahaley from PGE showed up on site and shut the valves on the production tanks to stop the release. A voicemail was left for Landon Vanord, Enviromental Technician from PGE, at 1033 on his cell phone making aware of the release. A plan as to how the spill will be remediated is being requested and should be submitted to the Department. - MRN (sw-viols corrected via IR 2017568)
2018737 Primary Facility 11/14/2011 Follow-up Inspection Outstanding Violations - No Viols Req'd A follow up inspection was conducted at the Fisk Hollow Pad B on 11/14/11 to collect samples to confirm the cleanup of a release of a pollutional substance discovered near the production tanks on 11/8/11 (2017568). A hook-up for pumping out the contained fluid was located off the containment and was leaking. On 11/10/11 PGE began the site cleanup. During this inspection, the contaminated soil was seen stored on plastic and was covered awaiting removal. There was a puddle in the excavated area showed a field meter reading over 16 mS. A soil sample was collected from the excavated area in front of the production tanks and submitted for SAC 960 analysis. A puddle in the collection channel at the toe of the slope below the production tanks showed a filed meter reading over 2,000 uS. A sample was collected and submitted for SAC 942 analysis. No new violations were cited during this inspection. - MRN

A hook-up for pumping out the contained fluid was located off the containment and was leaking. On 11/10/11 PGE began the site cleanup. During this inspection, the contaminated soil was seen stored on plastic and was covered awaiting removal. During this inspection, the excavated area remained open, the excavated soil was on-site and cover with plastic. The secondary containment for the three white production tanks, which were empty during this inspection, has been breached in order to excavated soil beneath the liner. A frac tank was on-site and production fluid was being stored there. Field meter reading for Specific Conductivity were taken from the puddles in the excavated area the ditch below the production tanks. No sample were collected because the surface of the puddle were frozen. No new violations were cited during this inspection.

During this inspection, the excavated has been backfilled. The secondary containment for the three white production tanks, which were empty during this inspection, is currently being reconstructed, the liner has not been re-installed as of this inspection . A mobile frac tank was on-site and production fluid was being stored there, on secondary containment. Field meter reading for Specific Conductivity were taken from the puddles around the excavated area and the ditch below the production tanks; they did not exibit elevated readings for Secific Conductivity. A pudlle behind the the secondary containment and white production tanks exibited a field conductivity reading of 1,900µS. This must be addressed. No new violations were cited during this inspection. 2045286 Primary Facility 02/24/2012 Compliance Evaluation Outstanding Violations - No Viols Req'd On Feb. 24, 2012 a Compliance Evaluation Inspection was conducted at the Fisk Hollow Pleasant Valley Pad B to observe the progress of the cleanup of a release of a pollutional substance discovered near the production tanks on 11/8/11 (2017568). The ground was frozen and snow covered. Weather conditions were 35 degrees and foggy. The site ID sign, site perimeter and the collection system for the production fluids were observed. There was no signs of erosion noted on-site. There was a puddle located in front of the production fluid tanks that exibited an elevated reading for specific conductivity (approx. 2mS). This was reported via email to PGE the afternoon of Feb. 24, 2012. PGE stated this would be investigated. No new violations were cited during this inspection. . - MRN

The excavation area had been backfilled and the were no puddles visible near the containemnt system. The following violations cited in the inpsection report (2017568) referenced above are being resolved.

2147824 Primary Facility 03/13/2013 Site Restoration No Violations Noted An inspection of the Fisk Hollow Clara Pad B in Clara Township, Potter County was conducted on March 13, 2013 in response to Well Site Restoration Reports received by the Department (for wells 2452, 2453, 2454, 2455, 2456 and 2457) . The last self-inspection conducted on behalf of PGE was on 3/12/13. The site has been re-graded and temporarily stabilized. the applicable Post construction Stormwater BMPs have been installed. The site was determined to not meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization at this time. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was not observed. A follow up inspection will be conducted at a later date to verify the site has been permanently stabilized. The Well Site Restoration Reports will not be approved until that time. No violations were cited during this inspection.
