2221377 Primary Facility 11/12/2013 Routine/Partial Inspection No Violations Noted A routine partial inspection of the Fisk Hollow Pleasant Valley Pad A site in Pleasant Valley Township, Potter County was conducted on 11/12/13, 2013 in response to a Notice of Termination and Well Site Restoration Reports received by the Department. The FH1949 (105-21302) well has been plugged. Six other wells are in production. The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization at this time. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. Other areas are stabilized with stone. The Well Site Restoration Report will be approved at this time. The approval of the Notice of Termination will be recommended. No violations were cited during this inspection.

2631715 Primary Facility 2017-08-29 Incident- Response to Accident or Event No Violations Noted On 8/29/17 at 1014 hrs I conducted an inspection at the Fisk Hollow Pad A well site to follow up on a small spill (less than 1 barrel) that was reported by PGE. It was mostly cloudy and 59 degrees F.

On 8/24/17 Amber Oyler, PGE Environmental Manager, contacted the Department of Environmental Protection (DEP) to report that approximately 25 gallons of hydraulic oil had been released to the pad surface at the Fisk Hollow pad A well site. She stated that she was notified of the spill at 1030 hrs on 8/24/17 by personnel at the site. At that time, absorbent pads were being used to soak up standing liquid. She stated that affected pad material would be removed and sent for disposal.

On 8/25/17, Ms. Oyler provided an update by email and included a photo to document that the spill had been cleaned up.

During my inspection, I observed the well pad and spill area. A new area of gravel was observed on the well pad to the southwest of the compressor building. It was apparent that pad soil had been removed in the spil

The wellhead assembly ,all fittings, flow lines and the cellar were tested for gas. The results of this testing was that no gas was detected on the wellhead assembly or the well area.
