The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The last inspection conducted on behalf of PGE was on 10/24/11. The pad has been constructed with no wells drilled at this time. The slopes have been stabilized and grass is currently growing. - MRN
2221688 Primary Facility 11/08/2013 Site Restoration No Violations Noted A routine partial inspection of the Fisk Hollow Pleasant Valley Pad D site in Pleasant Valley Township, Potter County was conducted on 11/08/2013 in response to a Notice of Termination received by the Department. No wells were drilled here and the site has been restored to original contours. The site is determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization at this time. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. The Well Site Restoration Reports will be approved. The approval of the Notice of Termination will be recommended. No violations were cited during this inspection.
