2609680 Primary Facility 2017-06-22 Routine/Partial Inspection No Violations Noted On 6/22/17 at 1345 hrs I conducted a routine inspection at the Sweden Valley 101 well site. It was overcast and 82 degrees F. Sean Bacha, Oil and Gas Consultant, was present at the site during the inspection.

The 2HU - 4HU wells were drilled in 2016 and are equipped for production. The 5HU well was spud, however, then plugged in December 2016. A new well permit application for the 6V well (105-21900) was received on 6/5/17.

I observed the E&S controls, production equipment and other materials staged on site. Grass is becoming well established on the topsoil stockpile and around the sediment traps; however, vegetative cover on the well pad fill slopes remains sparse. The sediment traps were observed (Photo #1). ST-1 (southern trap) held a minimal amount of water and had a field measured specific conductance of 209 micro Siemens/cm. The filter fabric had come off the rock filter and should be restaked (Photo #2). ST-2 (northern trap) was approximately half full and had a specific conductance of 73 micro Sie

2691252 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The wellhead assembly, all fittings, flow lines and vented annulus were tested for gas. Results of this testing was that gas was detected on the 13 3/8" x 9 5/8" vented annulus. No other gas was detected on or near the well area.
2691266 Primary Facility 2018-02-05 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. This is a follow up inspection from 1-25-18. This well was found to have measurable gas on the 13 3/8" x 9 5/8" vented annulus on the prior inspection and on this inspection as well. SGS Oil and Gas Chemicals Services gathered samples of this gas on 1-31-18. Results of this sampling were not known at the time of this inspection. No other gas was detected on the well or well area.
2706607 Primary Facility 2018-03-12 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being shut in for monitoring to watch and record well casing pressure build up. The well annulus 13 3/8"x 9 5/8" and 9 5/8" x 5 1/2" were being vented. The wellhead assembly,fittings,flow lines and annulus vents were tested for combustable gas. Results of this testing was that the 13 3/8" x 9 5/8" had 100% LEL (lower explosion limit) and 29% Methane measured on it with the gas meter. The 9 5/8" x 5 1/2" annulus vent tested no combustable gas. No other gas was detected on the wellhead assembly or the well area.
2707922 Primary Facility 2018-03-16 Routine/Complete Inspection No Violations Noted The purpose of this inspection was to observe isotopic sampling from the Intermediate #1 x Intermediate #2 annulus following a 4-dat shut-in test. The pad and surrounding are were covered in approximately 6 inches of snow at the time of the inspection. Rob Bowens and Sean Bacca (JKLM) were on location during the inspection. Prior to opening the vent for sample collection, 60 psi was observed on pressure gauge. A stainless steel tube was used to collect the sample by JKLM's consultant, SGS. No split samples were collected by the department. The annulus vent was left open following sample collection.
2715619 Primary Facility 2018-04-06 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being monitored every three(3) to four (4) days for a pressure reading on the 13 3/8" x 9 5/8" annulus vent. This vent was shut in during this time and then opened and pressure recorded. This will be an ongoing practice until a determination of a course of action is decide. Rob Bowens (production supervisor) was at this inspection.
2767307 Primary Facility 2018-08-24 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. Combustible gas was detected on the 13 3/8" x 9 5/8" annulus vent. Water was present in the cellar with no noted bubbling. This well had been monitored with periodic 72 hour shut in tests. At the time of this inspection it is not known if the shut in tests or monitoring is still ongoing. No violations were noted at this inspection.
2768967 Primary Facility 2018-08-23 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Sweden Valley 101 Well Pad ESG29-105-15-0003 on 8/23/2018 @1130 hours. The weather was 70 degrees sunny, 58% humidity and winds WNW 13 mph.

The 2HU-4HU wells were drilled in 2016 and are currently in production. The 5HU well was spud, then plugged in December 2016.

The access road, well pad and associated controls were inspected. A compressor unit appears to have been installed on the SE corner of the well pad. Large puddles were observed on the pad surface from recent heavy rains. All puddles on teh pad surface measured <300mS/cm(Photo#3). The fluid in teh duck pond containment had a field measured conductivity of 300uS/cm. JKLM representatives were contact and a vac truck was dispatched to the site. The elevated conductivity fluid was removed.

2804689 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. There are two (2) annulus vents openly venting on this well. Annulus vent intermediate x intermediate 2 (13 3/8"x 9 5/8") was venting 100% LEL (lower explosion limit) and 29% METHANE combustible gas. Although the Intermediate 2 casing string was not cemented to surface under an approved alternate method, combustible gas detected at this annulus indicates potential defective cement. JKLM should monitor combustible gas levels at this annulus on a monthly basis and report any significant changes to the Department as soon as possible. No other annulus vents were noted venting at this inspection. No violations noted at the time of this inspection.
2822376 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Sweden Valley 101 Well Pad located in Summit Twp, Potter Co. on 1/2/2019 @1115 hours. The weather 32 degrees, cloudy, 78% humidity winds, S @5mph. The site appears to be set up for production.

Empty production tanks appear to be stored around the perimeter of the site. Puddles observed on the pad surface were field screened for conductivity and found to be within normal range. No violations are noted at this time. We departed the site at 1200 hours.

2845851 Primary Facility 2019-02-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well as in production. A digital gauge was indicating a psi (pounds per square inch) of 1081. There are two annulus vents that are vented to surface and were tested for combustible gas with an Altair 5 x Multi Gas Meter. This gas meter was calibrated on 2-20-19. Results of this testing were 100% LEL ( lower explosion limit) and 4% Methane on the 13 3/8" x 9 5/8" vent and 0% LEL and 0% Methane on the 9 5/8" x 5 1/2" vent. No violations noted at this inspection.
