2542763 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No Chesapeake representatives were present.

Site well vegetated with greater than 70% perennial vegetation. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The cellars at the well heads were observed to be completely full of water. This fluid should be removed to prevent overflow.

No active drilling permits exist or are pending for this site. The most recent drilling permit (API No. 113-20344) expired on August 12, 2016. In order to remain in compliance with the Department's regulations pertaining to site restoration, a new drill permit should be obtained or an extension of time for site restoration should be requested.

No violations were cited during this inspection.

2563201 Primary Facility 2017-02-14 Routine/Complete Inspection No Violations Noted
2583296 Primary Facility 2017-04-17 Routine/Complete Inspection No Violations Noted Four wells in a production phase. Four each sand and water separators. No leaks. Site stable. No violations.
2724931 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted Fours wells staged for production phase. Four each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. Methanol tanks located near well heads. No violations noted.
2842611 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:26; the weather was ~15°F, calm, and sunny.

There were 4 properly tagged Marcellus wells staged for production. The cellars were filled with ice. There were 4 GPUs and sand separators onsite. Two produced water tanks were onsite and in secondary containment. A small section of corrugated pipe also remained on the northwestern edge of the pad Photo #2).

There was about 1 inch of snow covering the majority of the area. Much of the site had taller vegetation visible above the snow or had rock-lined slopes. However, 2 sections of the containment berm were cut through on the northeast and southwest edges of the pad. The visible portions of the areas appeared to be sparsely vegetated (Photos #3, 5). Additionally, a trench ran along the eastern edge of the access road and appeared to be eroding along portions of it (Photos #6, 7).

The Department recommends removing any waste materials and monitoring and reseeding/repairing the E&S concerns noted as weather permits.
