2464388 Primary Facility 2016-03-03 Routine/Complete Inspection No Violations Noted On 3/3/16, the Department conducted an inspection of the SWN RU53 Harris Pad while primarily conducting an inspection of the DTE/Susquehanna Gathering 53-54 Well Connect. The last E&S conducted at this pad was by Ryan Klemish on 3/2/16. The access road developed across the pad LOD for the pipeline construction, along with the pipeline LOD construction has good E&S controls installed. DTE has installed new controls which have been integrated into the well pads controls. Overall, E&S controls look good. All 6 wells associated with this pad are in production. No violations noted.

2495867 Primary Facility 2016-06-20 Routine/Complete Inspection No Violations Noted On 6/20/16, the Department met with representatives from SWN to inspect the damage to the retention berm of infiltration area #1 that resulted from the landowner breaching the berm. SWN contacted the Department on 6/15/16 to report the discovery of the damage to the berm. Among the SWN representatives were Mike Deming, Senior Regulatory Analyst, and Rich Gulyas, Construction Supervisor. Also present during the inspection was the land owner. The retention berm had an emergency spillway with a rock spreader and can be described as a small retention basin. The landowner explained that the fence that had been placed around the berm structure was breached by one of his bulls, and that his cattle began entering the basin. The landowner became concerned that his young cattle could enter the basin and possibly drown or get seriously injured. The berm is a permanent structure designed to function throughout the life of the well pad. The breach to the berm is in violation of the permit. This was explained to
2495896 Primary Facility 2016-06-22 Administrative/File Review No Violations Noted On 6/22/16, Mike Deming, SWN Senior Regulatory Analyst contacted the Department to report that the repairs to the infiltration berm that had been damaged by the landowner's cattle breaching the fence had been repaired. Mr. Deming sent a photograph of the repairs that clearly show the repairs have been completed. No further action is required of SWN at this time.
2560850 Primary Facility 2017-02-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 543psi on the production casing and 467psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:55a.m.

2655081 Primary Facility 2017-10-30 Site Restoration No Violations Noted This inspection was conducted to confirm SWN's assessment that the project meets the minimum requirements for permanent stabilization. The Departments determination is that the site meets the requirements for permit termination.
