Drillers log in mailbox indicated this well had 740' of 133/8 casing cemented to surface and 1618' of 95/8 casing cemented to surface. BOP was tested to 3000psi. The 2H had bop set on 95/8 casing. Drillers log indicated this well has 710' of 133/8 casing cemented to surface and 1614' of 95/8 casing cemented to surface. Yellow tag on head indicated hole was filled with 433 barrels of obm at 12ppg. Stack tested to 3000psi. The 3H had bop in place, yellow tag on stack indicating this well has 718' of surface and 1656' of 95/8 casing. Both tophole strings are cemented to surface. Hole was loaded with 433 barrels of 12 ppg OBM. BOP tested at 3000psi. |

The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. The weather conditions are sunny skies with temperatures in the low to mid 60's, with a slight breeze. There was no activity at the site. We met with Seth Tratthen, Regulatory Analyst of SWN. Mr. Tratthen stated the following in reference to the spill. "The clamshell on the centrifugal pump had a 3" crack in it. The pump leaked fluid into the containment and eventually overflowed onto the pad surface. The pump has been removed from the pad. The last containment inspection was on 9/27/17 and no issues were noted. The fluid spilled originated from the nearby SWN Zeffer large Impoundment. The fluid was a mixture of freshwater and production fluid pumped through an underground pipeline from the Zeffer to this pad to be boosted with the pump. A vacuum truck was on site on 10/3/17 and it recovered 50bbl (barrels) from the containment, ground, primary containment from the pump on the trailer, plumbing, and the risers.
