Chris Becker of Southwestern Energy contacted the Department at 10:30 AM on 8/3/15 to report that at approximately 9:30 AM on 8/3/15, a spill had occurred on the Range Unit 63; Mulloy Pad. The spill occurred while drilling of the 3H well was in progress. While drilling on air, the solids separation unit had a tank that overflowed (38) barrels of freshwater mixed with a polyacrylamide based flocculent. Due to compromised liner integrity in the containment, approximately (1) barrel of the fluid was released to the pad surface.

This report has been generated in response to remediation efforts conducted by SWN in response to a spill incident that had occurred at the site first documented on 8/4/15 by the Department in inspection record # 2398386. The Department concludes that the remediation of the spill conducted by SWN was adequate and that no further action is required of SWN in reference to this spill.

2535981 Primary Facility 2016-11-01 Incident- Response to Accident or Event No Violations Noted On 11/1/16 at 10:55AM, the Department arrived on site. The Department met with Dan Hessdorfer (Well Site Supervisor). Mr. Hessdorfer explained that he was present during the release which occurred at 2:30 PM. Mr. Hessdorfer stated that rainwater was in the containment and that there were four (4) spots that haven’t been properly welded. He also stated that the water appeared clear and that the containment was clean at that time. SWN was drilling the 2H well and waiting on cement at the time of the release. During the inspection, the Department noted that the SDC45 rig was on site and was drilling the 3H well.

We walked to the area of the release and found that there was orange spray painted arrows pointing to the breeches in containment. Mr. Hessdorfer explained that they were already patched. The Department screened water and soil in the vicinity (41¿ 51’ 11” -75¿ 36’ 27”) of the release and the readings were 0.20 mS and 0.014 mS respectively.

2540509 Primary Facility 2016-11-30 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:05a.m., I met with company man Rick Sizemore, SDC Rig 45 was on site and Schlumberger was running cement to cement the production casing into place. The top hole section of the well was drilled and completed on 8/22/15. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 11,875'. I left the site at 12:00p.m.

2562336 Primary Facility 2017-02-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20p.m., I met with company man Johnathon Davidson. Halliburton was on site and fracking the 2H, 3H, 4H, 5H, and 6H wells. They were pressuring up and preparing to frack stage 8 out of 21stages on the 6H. I took a walk around the location and there were no obvious concerns noticed.

The 2H was completed with 47 stages. The 3H was completed with 50 stages. The 4H was completed with 26 stages. The 5H was completed with 26 stages. I left the site at 12:55p.m.

2597564 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 15:05p.m.

The well was recently completed and is in production. The well tag was on the well and incorrect, I contacted Steve Moyer of SWN to have the permit number corrected on the well tag. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 589psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:50p.m.

The well was in production. The cellar was set with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
