2540483 Primary Facility 2016-11-23 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., Halliburton was on site and fracking the wells. I met with company men Dave Wilcox and Jonathan Davidson. They were planning on shutting down for the holiday and continuing back up on Friday. They completed stage 10 out of 19 stages on the 1H and the 2H, 3H, 4H, and 5H were completed. There were 18 stages on the 2H, 21 stages on the 3H, 23 stages on the 4H and 5H. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:20p.m.

2568368 Primary Facility 2017-03-07 Routine/Complete Inspection No Violations Noted The site was stable. The wells on this site have been recently flowed back and placed into production.
2578039 Primary Facility 2017-03-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:30a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 775psi on the production casing. There was a crew on site cleaning out two 500bbl tanks that were on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:25a.m.

2674620 Primary Facility 2017-12-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2695797 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Inspection conducted as a routine inspection. No E&S issues were identified. No violations noted.
2776470 Primary Facility 2018-09-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was containment around the five newest wells drilled and was unable to inspect the cellar. The well tender was on site when I arrived and he said tubing was recently put in the wells. The containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
