Gene Tyson is the Company Man on-site. According to Mr. Tyson, casing will be run on this well today and the casing will be cemented at (1,647) feet. The surface casing was set at (603) feet. The well will be drilled to kickoff point at (5,950) feet. Falcon 25 will drill the 2H and 5H wells to kickoff point. The 4H well is in production. SDC Drilling Rig#44 will drill the 1H, 2H and 5H from kickoff point to total depth.

The top hole section of the well was drilled and completed on 12/8/15. I took a walk around the location and everything looked OK.

Total Depth of hole at time of inspection was 14808'. They were at 11833' with the 5.5 production casing. I left the site at 13:20p.m.

2521332 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:50a.m., Halliburton was on site to stimulate the 1H, 2H, and the 5H wells. I met with Mark McPherson (Workover and Completions Forman). They were starting stage 19 out of 35 stages on the 1H.

The 2H was completed with 32 stages. The 5H was completed with 22 stages. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:20a.m.

2586445 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:30p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 468psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:10p.m.

2691115 Primary Facility 2018-02-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below ice and was unable to check for gas. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2791599 Primary Facility 2018-10-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
