I was on site to conduct a new spud inspection for the 5H (115-22321) and 6H (115-22322) wells. There was no equipment on site. The conductor pipe has been drilled and set. I checked the drillers log book to get the depth of the conductors. The conductor pipe was set at 75' on the 5H and 6H wells. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2742431 Primary Facility 2018-06-18 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Robert Gray. Falcon 24 was on site and drilling the vertical section of the well. The rig is currently waiting for a drill bit. There is one well on the pad that is in production. They are drilling the second well out of two to be drilled. The proposed TD for the Top-Hole section of the well is 5000'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2752150 Primary Facility 2018-07-16 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Carl Johnson. SDC 44 was on site and drilling the lateral section of the well. They were pulling the drill pipe out of the hole because of a motor failure when I arrived on site. There is one well on the pad that is in production. They are drilling the second well out of two to be drilled. The proposed TD for the well is 19,269'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2772890 Primary Facility 2018-09-10 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company men Mark McPherson and Jonathan Sites. USWS was on site to frac the 5H and 6H wells. They were stimulating stage 11 out of 64 stages on the 5H and preparing to run a plug and perf in the 6H when I arrived on site. There were 12 stages completed out of 64 on the 6H. The water was being pumped to the location from the Zeffer production fluid impoundment and the Hinkle extraction, it was also being trucked to the site from the Buckhorn pad that recently had three wells stimulated and other nearby well pads. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2815806 Primary Facility 2018-12-14 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 3 wells appear to be in production. No E&S issues identified. Good vegetative and stone stabilization present. No violations present.
2821626 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The permit numbers on the entrance sign were correct. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
