Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2669403 Primary Facility 2017-12-11 Drilling/Alteration No Violations Noted At time of inspection.

On site for a new spud inspection. Minichi Contracting Group was on site to drill and set the conductor pipe on the 1H, 2H and 4H wells. I met with driller Ralph Dewald. The rig was on the 2H at a depth of 125'. The 1H and 4H wells were drilled and set at 135'. The drill cuttings are put in a role off container and hauled to the Keystone landfill. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2687669 Primary Facility 2018-01-29 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Robert Gray. Falcon 24 was on site and drilling the top-hole section of the hole. Proposed TD for the top-hole section of the well is 5600'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2717436 Primary Facility 2018-04-12 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Tommy Howard. SDC 44 was on site and was drilling the lateral section of the well. The top hole section of the well was drilled and completed by Falcon 24 on 1/31/18. The proposed TD for the well is 12,916. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2733561 Primary Facility 2018-05-23 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Derek Johnson. USWS was on site to frac the 1H, 2H and 4H wells. They were on stage 20 out of 29 on the 1H. There were 23 stages completed out of 33 on the 2H and they were running the plug and perfs when I arrived on site. There were 13 stages completed out of 23 on the 4H. The water was being pumped to the site from the Zeffer production fluid impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2755831 Primary Facility 2018-07-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2820610 Primary Facility 2018-12-28 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 4 wells appear to be in production. No E&S issues identified. Good vegetative stabilization present. No violations present.
