2571274 Primary Facility 2017-01-31 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Charles Johnston well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetative cover no erosion was observed. The well heads were painted and maintained no leaks or fluids were observed around the well heads. Production tanks were in good condition no leaks, rust or defects observed. Secondary containment for the site was intact and maintained.

9H is Active

2698519 Primary Facility 2018-02-22 Administrative/File Review No Violations Noted On site inspection prior to granting inactive status. All three wells appear shut in. On site inspection shows no apparent violations. Wells are in an area with numerous pipelines being installed which supports operators claims for returning well to production when sufficient capacity exists. NOVIO
