2695433 Primary Facility 2018-02-16 Routine/Partial Inspection No Violations Noted SWEPI 's inspection logbook is in a mailbox near the site ID sign; last inspection listed on February 3, 2018. Electrical conductivity (EC) readings taken with an Oakton PCSTestr 35. Air temp 36F. JLP
2798066 Primary Facility 2018-10-30 Complaint Inspection No Violations Noted Weather: sunny with few clouds; 60 degrees F; low humidity; slight to moderate wind from west to east. This inspection was conducted in response to an odor complaint received at 1255 on 10/30/2018. A distinct chemical/hydrocarbon odor was observed upon arrival to the site at 1400 on 10/30/2018. An Altair 5X combustible gas meter was used to screen air quality (calibrated 10/24/2018) during the inspection. Based on wind direction and odor observed during the inspection, the source of the odor appeared to be the reboiler on the dehydration unit located inside the fenced area of the compressor station. Air quality monitoring indicated 0% LEL; 0% Methane/Vol; 0 ppm H2S; 0% CO; and 20.9% O2 with no changes detected for the duration of the inspection.

The six wells on location appeared capable of production. One 2-inch diameter annulus vent was observed at each well during the inspection. No gas was observed at any of these annulus vents or in the immediate vicinity of the gas wells. No violations were noted duri

2826279 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Parthemer 284 Well Pad in Charleston Twp, Tioga County on 1/14/2019 at 1040 hours. The inspection was conducted to assess site conditions. The weather was sunny and 21 degrees.

There are currently 6 wells onsite capable of producing with 6 production tanks. One well appears to be flowing back and a vertical separator and frac tank are installed on it. The site is well vegetated and remains stabilized. No erosional issues were observed today and all fluids on pad surface were frozen solid.

No violations noted today, I departed the site at 1105.

2839233 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Cold snowy/windy at time of inspection
