The last inspection conducted on behalf of the operator was on August 19, 2013. 1H, 2H, 3H, 4H, 5H and 6H are drilled and in production. The site is well vegetated and stabilized. No puddles exhibiting elevated conductivity were observed at this time. The Post Construction BMPs listed in the E&S plan have been installed. The Restoration reports for the 1H through 6H well are being approved. No violations were cited during this inspection.

2324363 Primary Facility 11/21/2014 Site Restoration No Violations Noted The Department inspected the Matz 824, Well Permits #117-20600, 117-20615, 117-20711, 117-20712, and 117-20713, 117-20714 in Chatham Township, Tioga Co. on 11/21/14 @ 1330 hours. The weather was 27 degrees, 16mph wind gusts out of the NW, with 54% humidity. The Well Site Restoration Report was received by the NCRO on November 13th, 2014. This inspection complies with 25 PA Code §102.22. No violations issued during this inspection. Approval of the Notice of Termination is recommended at this time. The Well Site Restoration Report for permits# 117-20600, 117-20615, 117-20711, 117-20712, 117-20713, and 117-20714 is approved at this time.
2443204 Primary Facility 2016-01-20 Routine/Complete Inspection No Violations Noted The Department inspected the SWEPI LP site, the Matz 824 located in Chatham Twp., Tioga Co. on 1/20/2016 @1300 hours. The weather was 27°, cloudy and blustery. The total project area is 33.31 acres. The receiving stream is listed as UNT to Crooked Creek/Susquehanna River Basin in PA, WWF designation. The E&S inspection log showed the last inspection was completed 4/23/15, with “no changes” listed in the comments section. The well tags are visible. The pad surface measures 250' x 150'. The permanent E&S controls along the haul road were observed and appear to be functional. Three baffled tanks were observed onsite in containment. Fluid in the tank containment was frozen solid. No violations were noted during this inspection. I departed the site @1345 hours.
2538247 Primary Facility 2016-11-14 Routine/Complete Inspection No Violations Noted The Department inspected the SWEPI LP site, the Matz 824 located in Chatham Twp., Tioga Co. on 11/14/2016 @1200 hours. The weather was 55°, partly cloudy. The total project area is 33.31 acres. The receiving stream is listed as UNT to Crooked Creek/Susquehanna River Basin in PA, WWF designation. The E&S inspection shows no recent inspections since the ESCGP has been terminated.

Cut and fill slopes appear well vegetated and stabilized. Well tags were observed on the well heads. One roll-off container was observed in containment. No fluid was observed in the secondary containment around the produced fluids tanks. No violations were noted during this inspection. I departed the site @1345 hours.

2856562 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Matz 824 Well Site in Chatham Twp, Tioga County on 3/19/2019 at 1230 hours. The weather was partly cloudy and 40 degrees. The act 9 sign was observed at the entrance to the site. The E&S Plan was in the mailbox at the entrance. The onsite inspection log was in the mailbox by the production tanks. Company personnel last inspected the site on 3/16/2019, noting no issues.

There are currently 6 producing wells onsite (1H-6H). The site is very well vegetated and stabilized. No accelerated erosion was noticed onsite. Containment structures were intact and functioning. No evidence of spills or releases could be found. PCSM BMPs are installed and functional. Puddles on the pad surface were field measured within normal conductivity ranges.

No violations noted today, I departed the site at 1315 hours.
