2446714 Primary Facility 2016-02-02 Site Restoration No Violations Noted The Department of Environmental Protection inspected the SWEPI LP – Fuller 891 site in Deerfield Twp., Tioga Co on 2/2/2016 @ 1230 hours in response to receipt of a Notice of Termination and Well Site Restoration Report by the Williamsport District Office on 11/12/2015. The weather was 45 degrees and sunny. The onsite inspection log was located in the mailbox along with a copy of the well permit. The site was last inspected by company personnel on 1/26/2016. The comments section in the onsite inspection logs for 12/3/2015 states “sed sock needs to be removed”.

As of today’s inspection, the sediment sock below the fill slope is still in place. The site does not meet the requirements of 70% vegetative cover. A small erosion channel has formed below a rock apron on west side of the access road that needs to be addressed. The erosion channel was previously noted in the onsite inspection log. Based on the site conditions present during today’s inspection, it is recommended this NOT be returned. The WSRR will be
