2531850 Primary Facility 2016-10-24 Routine/Complete Inspection No Violations Noted The Department inspected the SWEPI LP – Neal 815 well pad (ESX09-117-0087) in Chatham Twp., Tioga Co. on 10/24/2016 @ 1245 hours. The weather was 31 degrees and sunny. An adequate site ID sign was observed at the entrance to the access road. The E&S plan, the PPC and the onsite inspection log were located in the mailbox onsite. The receiving stream was listed as UNT to Crooked Creek, WWF. The site was last inspected by company personnel on 10/22/2016 with the following comments “Erosion starting on front side of pad and in corners. Sed trap draining into neighbors back yard”

As of today’s inspection, the site appears to have been restored recently. ST1 and ST2 have been replaced by a large infiltration basin below the fill slope of the pad. Due to recent above average precipitation events multiple areas of erosion are visible on the sidewalls of the basin and off the corners of the pad. Please see attached photos. 2559410 Primary Facility 2017-02-01 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP Neal 815 well site in Chatham Twp, Tioga Co. on 2/1/2017. The weather was cloudy, 30 degrees, fresh snow accumulation of 3". No activity observed onsite. No puddles observed on the pad surface. No vegetation was visible, but the site appeared stable. Erosion and sediment controls were not visible due to snow cover, but no issues were observed. No violations were issued during this inspection. Please contact me with any questions.

2671836 Primary Facility 2017-12-15 Routine/Partial Inspection No Violations Noted Spoke with Heath McAfee (SWEPI OSR) on site. He said Ensign 162 started drilling the 1st of the new Utica wells (24H) on December 4, 2017. No electrical conductivity readings taken today, as the site is frozen.
2722262 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the Neal 815 Well Site located in Chatham Twp, Tioga Co. on 4/24/2018 @ 1300 hours. The weather was 61 degrees and partly sunny. Phil Tillitt, SWEPI OSR and Justin Midgley, SWEPI Engineer were present onsite for the inspection.

Shell is in the process of fracking 4 Utica wells on the pad, the 21H, 22H, 24H and 26H. The rack is scheduled to be complete in 4 to 5 days. Water was observed in the ditches surrounding the pad. Puddles on the pad surface were tested for conductivity and found to be in normal range. The Infiltration basins below the fill slope of the pad were holding water.

CTS#333786 can be closed out at this time.

No violations are noted at this time. I departed the site at 1415 hours. -ELN

2770317 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP Neal 815 Well Pad in Chatham Twp, Tioga County on 8/28/2018 at 1100 hours. The weather was clear, 55% humidity and 85 degrees. Ellie Niles, DEP Water Quality Specialist, was onsite with me.

The drilled Utica Wells onsite are currently flowing back while the Marcellus wells are producing. Company personnel man was onsite monitoring the flowback. We spoke to him and a well tender that showed up about the elevated conductivity in the puddles and they said it would be taken care of. We later arrived onsite at 1500 hours on the way back to the office and the puddles had since been cleaned up (see photos 9 & 10).

The violation that was immediately corrected was against the ESCGP# ESX091170087(3). The elevated conductivity in the puddles is considered to be in violation to 25 Pa Code 78a 56(a)-failure to contain a regulated substance generated at or used at a well site. This violation is considered immediately corrected because it was remediated in a timely
