Per § 3216(c) of the 2012 Oil and Gas Act (58 Pa.C.S. §3216(c)), site restoration is required to occur within 9 months of completion of well drilling, unless an extension request is filed with, and approved by the Department. A restoration extension request was approved by the Department on 04/09/2015, which extends the restoration deadline until 04/09/2017.

2543584 Primary Facility 2016-12-08 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No SWEPI representatives were present.

Vegetative cover on site is greater than 70%. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

A restoration extension request was approved by the Department on 04/09/2015, which extends the restoration deadline until 04/09/2017. The Erosion and Sediment Control General Permit for the site expires on 05/01/2017.

No violations were cited during this inspection.

2717059 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted Weather was 33° and sunny at time of inspection. Thrre were no Rockdale representatives on location at time of inspection. Well had one analog pressure gauge attached above the swab valve and one analog pressure gauge attached to the 5 1/2 inch casing. The pressure gauge attached above the swab valve read approximately 740 psi and the gauge attached to the casing read approximately 250 psi at time of inspection. There were three annulus vents plumbed to surface. Unable to determine the origin point of each individual annulus vent due to the cellar being full of gravel. Annulus vents were 2", 1 1/2" and 1". No combustible gas was detected at time of inspection. No violations noted at time of inspection.
2732016 Primary Facility 2018-05-14 Routine/Complete Inspection No Violations Noted At the time of the inspection; containment, tanks, and equipment were installed in anticipation of completions activities on several of the wells onsite. PCSM/Site Restoration plans have been partially implemented. Infiltration basins have been installed but the impervious gravel pad surface has not been reduced as depicted in the approved plans. A 2-year extension on site restoration was approved on 10/20/2017. The Department recommends the PCSM/Site Restoration plans be implemented as permitted by 10/20/2019 or modified to account for the impervious pad surface.
2738628 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted I met Luke Chesner and Levi Lee of Rockdale when I arrived. Coil tubing rig on location drilling out plugs on the 6H prior to moving to the 4H. I noted two areas on the pad that were wet and muddy, one between the coil tubing rig and a pump truck (approximately 5-8 gallons of free water and 30' x 40'), and one between two Baker tank batteries (approximately 10-15 gallons of free water and 30' x 50'). Conductance reading in the former was 9.22 mS/cm, and was over the limit of my meter (19.99mS/cm) in the latter. I pointed both of these areas out to Luke and Levi, and took a grab sample (ID#0939066) from a puddle in the area between the Baker tank batteries. Luke and Levi had a vac truck working in the area between the pump truck and the coil tubing rig almost immediately, and I recommended that they vac the area between the Baker tank batteries and start to remove the affected soil ASAP. The elevated conductance in these areas is an indication of a potential release of produced water or flowback to the
2761125 Primary Facility 2018-08-06 Incident- Response to Accident or Event No Violations Noted I arrived on site in response to a spill notification from Rockdale called in at 09:45 on 8/4/18. Regina Logue of Rockdale reported that approximately 2 barrels of potentially contaminated rainwater was released to the pad due to a failed containment, and stated that the release was near the aboveground storage tank. The only person present at the pad when I arrived was Brady Bryington of Principal Enterprise. He pointed out the area of the release, and when I inquired informed me that the AST contained fresh water. I screened the release area, which was at the corner of an upright tank containment, with my soil conductance meter; most readings were in the range of .10-2.2 mS/cm, and the readings dropped dramatically when I pushed the meter 1/4" into the soil. I did locate a spot that seemed to be somewhat removed from the spill area near the center of the upright tank containment that was over limit on my soil meter (greater than 4.0 mS/cm).
