2683972 Primary Facility 2018-01-18 Routine/Partial Inspection No Violations Noted I spoke with Bret McCoy (SWEPI OSR). He expects to be drilling the Utica wells (4) here for another 45 days. JLP
2695152 Primary Facility 2018-02-15 Routine/Partial Inspection No Violations Noted I spoke with Bret McCoy on site who explained they are at ~5,220' on the final Utica Shale well of the 4 planned for this round of drilling. They are using brine water as drilling liquid. Chris Harrison (Halliburton) explained the salt (photo) is NaCl, which makes 10lb/gallon brine at full saturation. They plan to switch to synthetic based drilling mud when the get to ~6,500'.

Highest electrical conductivity (EC) was 1.5 mS/cm from a puddle in the northeast corner of the pad, and 2.9 mS/cm from liquid on the rail system for the open top dumpsters at the drill cuttings shakers (on containment). I used an Oakton PCSTestr 35 for the EC readings. JLP

2712751 Primary Facility 2018-03-30 Routine/Partial Inspection No Violations Noted I spoke with Dean Albright (SWEPI) on site: Keane is hydraulically fracturing the 4 Utica Shale wells on site. Freshwater is piped from the Cummings impoundment.

I got electrical conductivity (EC) readings with an Oakton PCSTestr 35; the highest reading off containment was 1.7 mS/cm on the east side of the pad (photo 3). SWEPI inspection logbook is in the mailbox at the production tanks; the last inspection is listed on March 24, 2018, and it states that woodchucks are digging into the pad. Air temp 38F. JLP

2722290 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the Neal 815 Well Site located in Chatham Twp, Tioga Co. on 4/24/2018 @ 1300 hours. The weather was 61 degrees and partly sunny. Phil Tillitt, SWEPI OSR and Justin Midgley, SWEPI Engineer were present onsite for the inspection.

Shell is in the process of fracking 4 Utica wells on the pad, the 21H, 22H, 24H and 26H. The rack is scheduled to be complete in 4 to 5 days. Water was observed in the ditches surrounding the pad. Puddles on the pad surface were tested for conductivity and found to be in normal range. The Infiltration basins below the fill slope of the pad were holding water.

CTS#333786 can be closed out at this time.

No violations are noted at this time. I departed the site at 1415 hours. -ELN

2770352 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP Neal 815 Well Pad in Chatham Twp, Tioga County on 8/28/2018 at 1100 hours. The weather was clear, 55% humidity and 85 degrees. Ellie Niles, DEP Water Quality Specialist, was onsite with me.

The drilled Utica Wells onsite are currently flowing back while the Marcellus wells are producing. Company personnel man was onsite monitoring the flowback. We spoke to him and a well tender that showed up about the elevated conductivity in the puddles and they said it would be taken care of. We later arrived onsite at 1500 hours on the way back to the office and the puddles had since been cleaned up (see photos 9 & 10).

The violation that was immediately corrected was against the ESCGP# ESX091170087(3). The elevated conductivity in the puddles is considered to be in violation to 25 Pa Code 78a 56(a)-failure to contain a regulated substance generated at or used at a well site. This violation is considered immediately corrected because it was remediated in a timely
