There are 6 producing wells on this pad. Both annulus vents are closed and not vented at time of inspection. Production by intermediate (5-1/2¿ X 9-5/8¿) annulus vent is shut in to pressure monitor, with pressure reading ¿ 0 PSI, production casing annulus vent is also shut in to pressure monitor, with pressure reading -210 PSI. The production xmas tree top pressure on the 2-3/8¿ tubing is 153 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2760254 Primary Facility 2018-07-25 Routine/Complete Inspection No Violations Noted On July 25, 2018, Department representative Sue Herzing conducted a routine site inspection at COP TRACT 285 PAD B. The site sign, emergency response plan, monthly tank maintenance log, ESCGP, weekly E&S inspection log, and drilling permits were onsite for review. No E&S issues were observed during the time of my inspection. A PCSM infiltration berm consisting of a 24" seeded compost sock has been installed according to PCSM plans. This compost infiltration berm is well vegetated and appears to be functioning as design. The pad remains the original footprint size of 400' x 400'. According to the operators PCSM plans, this the smallest "in production" workable footprint size. The following remain in place along the access road and appear to be functioning as design: rock lined roadside ditches and ditch relief culverts with rip-rap aprons. Overall, the site is stable and well vegetated exceeding 70% uniform, perennial, vegetative cover.

2845170 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted Inspection was in response to a deficient tank notification from ARD received on 2/25/19. A hole was discovered above the water level, and the tank was taken out of service and pumped down. I immediately noted several rust streaks on the #1 tank, and also noted that the #3 tank had similar rust streaks; I recommend that the #3 tank be monitored carefully for integrity. 3-4" of ice was present in the production tank containment; although the capacity of the containment did not appear to be significantly reduced, I recommend that when conditions warrant that this containment be vacced out. The GPU containment was full to the top of the berm with ice; I recommend that this be vacced as soon as reasonably possible.
2855419 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Weather was 20 degrees and sunny. There were no Alta representatives on location at time of inspection. Well was in production. Well had digital pressure monitoring equipment. Well had 136 PSI on the production tubing. Well had a 0 to 10k PSI analog pressure gauge on the production casing reading approximately 500 PSI. Well had 2 visible annulus vents plumbed to surface. 1' annulus vent had a 0 to 600 PSI analog pressure gauge reading approximately 0 PSI. 2' annulus vent had no gauge to read pressure. 0% combustible gas was detected around the well. No violations at time of inspection.
