There are 6 producing wells on this pad. Both annulus vents are closed and not vented at time of inspection. Production by intermediate (5-1/2¿ X 9-5/8¿) annulus vent is shut in to pressure monitor, with pressure reading ¿ 0 PSI, production casing annulus vent is also shut in to pressure monitor, with pressure reading -162 PSI. The production xmas tree top pressure on the 2-3/8¿ tubing is 149 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2760243 Primary Facility 2018-07-25 Routine/Complete Inspection No Violations Noted On July 25, 2018, Department representative Sue Herzing conducted a routine site inspection at COP TRACT 285 PAD A. The site sign, emergency response plan, monthly tank maintenance log, ESCGP, weekly E & S inspection log, and drilling permits were onsite for review. No E & S issues were observed during the time of the inspection. PCSM infiltration berms and diversion berms have been installed per the PCSM drawings. These berms consist of seeded compost sock of various sizes. Vegetation on the socks is spotty but the socks are serving their purpose. The well pad remains its original footprint size. The following remain in place along the access road: rock lined roadside ditches and ditch relief culverts with rip-rap aprons. Overall, the site is stable and well vegetated exceeding 70% uniform, perennial, vegetative cover.

There are a total of 4 producing wells located on this well pad and 1 plugged well (035-20276). All wells are tagged with API/well permit number. All well cellars are full of water. No bubbl

Well was in production. Well had digital pressure monitoring equipment that had no read out. Well had a 2' annulus vent plumbed to surface with no gauge to read pressure. Well had a 1' annulus vent plumbed to surface with a 0 to 300 PSI analog pressure gauge reading approximately 0 PSI. 0% combustable gas was detected in any area around the well at time of inspection. Well had a 0 to 10k PSI analog gauge on the production tubing reading approximately 100 PSI. No violations noted at time of inspection.
