There are 6 producing wells on this pad. Both annulus vents are closed and not vented at time of inspection. Production by intermediate (5-1/2¿ X 9-5/8¿) annulus vent is shut in to pressure monitor, with pressure reading ¿ 0 PSI, production casing annulus vent is also shut in to pressure monitor, with pressure reading - 283 PSI. The production xmas tree top pressure on the 2-3/8¿ tubing is 200 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. 2768928 Primary Facility 2018-08-21 Routine/Complete Inspection No Violations Noted The weather is 65 Deg. F and cloudy at the time of inspection. Routine inspection of Alta Resources, LLC., C.O.P. TRACT 285 E Pad, Well # 1029H, on August 21, 2018. There are 6 active and producing wells on this pad. Annulus vent was open and shut into the pressure gauge at time of inspection. 0% combustible gas was detected in the area immediately surrounding the well.

2820804 Primary Facility 2018-12-21 Routine/Complete Inspection No Violations Noted On 12-21-18, Department representative Brad Kline and Jared Jacobini conducted a routine site inspection at pad COP TRACT 285 E. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 6 producing wells located on this well pad. All wells are tagged with API/well permit numbers. Bubbling within the cellar was observed at well 035-21228 at elbow (See photo). The bubbling issue was referred to the OGI covering the area. There are 6 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment. Production tanks had some minor corrosion and staining. The Department recommends monitoring the condition of the production fluid tanks.
