2695818 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted The weather is 36 Deg. F and sunny at time of inspection. Routine inspection of Alta Resources, LLC., C.O.P. TRACT 285C Pad, Well #1018H, on February 13, 2018.

There are 6 producing wells on this pad. Both annulus vents are closed and not vented at time of inspection. Production by intermediate (5-1/2¿ X 9-5/8¿) annulus vent is shut in to pressure monitor, with pressure reading ¿ 0 PSI, production casing annulus vent is also shut in to pressure monitor, with pressure reading -230 PSI. The production xmas tree top pressure on the 2-3/8¿ tubing is 198 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2807840 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted On November 28, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 285 Pad C. Site stability and site restoration could not be evaluated due to snow cover.

There are a total of 6 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There is a sand separator associated with each well. There are 6 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted.

2855392 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Weather was 20 degrees and sunny. There were no Alta representatives on location at time of inspection. Well was in production. Well had digital pressure monitoring equipment that had no read out. Well had one visible 2' annulus vent plumbed to surface with a 0 to 5k PSI analog gauge reading approximately 500 PSI. 0% combustable gas was detected in any area around the well at time of inspection. Well had a 0 to 10k PSI analog gauge on the production tubing reading approximately 200 PSI. Well had a 0 to 10k PSI analog gauge on the production casing reading approximately 500 PSI. No violations noted at time of inspection.
