2825683 Primary Facility 2019-01-11 Compliance Evaluation No Violations Noted On 1/11/19 a compliance evaluation inspection of the Bare Site was conducted in response to violations noted on 12/13/17. The following was observed:

>>No residual impact was found in the area of the spill.

>>An accessory pad on the site was handling heavy truck traffic with many parked and idling trucks and trailers. The pad was poorly stabilized with substantial mud and tracking on site. No impact was observed off pad or on nearby roadways. Improvement of pad surface recommended.

2831569 Primary Facility 2019-01-25 Drilling/Alteration No Violations Noted Falcon Drilling Rig #29 moved onto this well on 1/12/19 and resumed drilling at 131 ft. KB on 1/15/19. Note: 20" conductor casing was set at 120 ft. on this well and there is no separate water-protective casing. The 13 3/8" combined water / coal casing was set at 309 ft. On 1/16/19 and cemented to surface with 12 bbls returned. The 9 5/8" casing was set at 1477 ft. on 1/18/19 and cemented to surface with 26 bbls returned. TD was at 4725 ft. on 1/21/19.
2831732 Primary Facility 2019-01-25 Routine/Complete Inspection No Violations Noted At the time if inspection the Falcon Drilling rig #29 and actively drilling API 125-28637. The drill rig was placed on containment. The Act 9 site ID sign was installed. The PPC plan, ER plan and waste control and disposal plan were on site. The site was covered with approximately 2 inches of snow make the assessment of accelerated erosion or sedimentation difficult. Operator Rep. Lee Mitchell indicated that the well has been drilled to the kick off point.
2850224 Primary Facility 2019-03-07 Drilling/Alteration No Violations Noted Patterson 288 moving onto location in anticipation of drilling four downhole sections. Operator anticipates rig in operation early next week. On site inspection found no apparent violations. NOVIO

2856113 Primary Facility 2019-03-20 Routine/Complete Inspection In Compliance with Policy Inspection related a release reported by the Operator on 3/15/2019.

The Operator reported "On 3/15/2019 at 7:30 PM, while drilling ahead on the 21H, a failure occurred on the kelly hose. Subsequently, 10 gallons of oil based mud (OBM) released to containment and 10 gallons misted onto the graveled well pad and adjacent equipment. The OBM impacted gravel was immediately scraped up and placed into roll-off boxes. Equipment that OBM misted onto was wiped clean. The onsite vac system was utilized to recover the released OBM that remained in containment. Disposal documents will be sent to you as part of the closure summary."

At the time of inspection the area impacted by the release was excavated and backfilled with rock. The impacted material was placed in a roll off boxes awaiting disposal.

The Operator is in compliance with the Departments spill policy.
