Lease tender Bobby Hamel was onsite pumping rain water out of the containment around the produced water tank. All well cellars were filled with gravel. There were two open annular vents at each well. The 13-3/8” x 9-5/8” was vented with a 1” pipe. The 9-5/8” x 5-1/2” annulus was vented with a 2” pipe. The annuli were monitored with an Altair 5X gas meter calibrated on 12/28/15.
2543932 Primary Facility 2016-12-08 Routine/Complete Inspection No Violations Noted Routine Inspection; 12/8/2016. I entered the location at 10:06 and began the inspection. At the time of the inspection, the Falk Unit 1H (081-20144), 2H (081-20180), 3H (081-20205), 4H (20209) and 5H(081-20210)wells were in production. The well pad area was relatively well vegetated but some erosion was noted along the access road and one of the basins onsite was had a fair amount of standing water. The well site has not been restored and the Well Site Compliance Information Request reply (submitted 9/21/2015) says an ESCGP-2 was to be submitted in the fall of 2015. The well pad remains at its original size and the site currently does not conform to the permitted restoration plan. The original E&S permit has been expired since 10/16/2014.
 The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:
1. All permanent post construction stormwater control features as identified in the Post Construction
