Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2132720 Primary Facility 01/22/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was in the process of cutting and removing trees and underbrush. The inspection at the request of the Operator to inspect perimeter BMP controls prior earth disturbance. At the time of inspection the Erosion and Sedimentation plan was on site and implemented as designed. No observation of accelerated E&S issue in relation to proposed well pad area or the access road. The Department suggests that the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2139197 Primary Facility 02/13/2013 Routine/Complete Inspection No Violations Noted no violation noted
2152934 Primary Facility 04/02/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was in the process of constructing the site.
2163797 Primary Facility 05/06/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was in the process of constructing the site.
2174949 Primary Facility 06/13/2013 Routine/Complete Inspection In Compliance with Policy Inspection the result of the Operator reporting a breached BMP during a precipitation event. The Operator is in compliance with the Departments notification policy for a BMP breach. During the inspection the Department notes that an infiltration ditch located closest to the access road failed during a heavy precipitation event in the early morning hours on 6/13/2013. The failure resulted in material from the infiltration ditch being washed into a 32" silt sox, which did not contain the material and also failed. The two failures resulted is accelerated erosion of the access road and scouring of a hillside below the excavated pad area. The precipitation event appears to exceed the Departments criteria for construction standards. The Operators response was to rebuild the failed portion of the infiltration ditch, add an additional rock outlet and repair the silt sox. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2175881 Primary Facility 06/13/2013 Drilling/Alteration No Violations Noted Well not spud in at time of inspection. Inspection conducted w/OGI J. Branan.
2177609 Primary Facility 05/24/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Department notes that the Operator replaced the silt sox, installed an additional rock outlet, and repaired the failed earthen dike surrounding the well site. The Department has also notes that the additions have been added to the E&S plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2182012 Primary Facility 07/05/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. No observation of accelerated erosion or sedimentation in relation to the access road or well site. The E&S plan was on site and all associated BMPs appear to be installed as outlined on the plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

2185972 Primary Facility 07/24/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Operator indicated that the well was drilled to the KOP. See inspection #2185968 for description of observations made during site inspection. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2189850 Primary Facility 08/01/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Operator indicated that the well was drilled to the KOP. See inspection #2185968 for description of observations made during site inspection. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2189853 Primary Facility 08/06/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Operator indicated that the well was drilled to the KOP. See inspection #2189814 for description of observations made during site inspection. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

2249710 Primary Facility 03/03/2014 Routine/Complete Inspection No Violations Noted See inspection # 2249702 for events and description from the 3/3/2014 inspection.
2253170 Primary Facility 03/13/2014 Incident- Response to Accident or Event No Violations Noted Incident inspection the result of a 2 gallon motor oil spill reported by the Operator on 3/12/2014. The Operator is in compliance with the spill policy. During an onsite inspection the Operator indicated the spill occurred during a maintenance break. The Operator indicated that a mechanic spilled the motor oil while preforming routine maintenance on an engine located on top of containment. The continent was full of precipitation and the motor oil was suspended on the water, which results in a portion of the oil reaching a portion of the pad.
