2151167 Primary Facility 03/21/2013 Routine/Complete Inspection No Violations Noted Well is drilled. Well tag is wrong, says Prevost. Clean out cellar and around cellae. Permit # on site sign is illegible. Immediately corrected.
2272023 Primary Facility 05/14/2014 Routine/Complete Inspection No Violations Noted Stone Well Services Rig # 61 was on location in the process installing production tubing for the Nardini 1H well. The Nardini 1H well is in production. A vacuum truck was on-site in the process of unloading production fluid. All wells at the P&G location location have been hydraulically fractured. Frac plugs have been drilled out for the Prevost 1H and Ruark 4K wells. Flowback procedures are set to commence shortly for the Prevost 1H and Ruark 4H wells. I recorded (1,300) microSiemens/cm conductivity in water on the frac tank battery containment liner behind the frac tanks in the northwest corner of the Well Pad. A vacuum hose to remove standing water on the surface of the Pad and containment areas of the site, was staged on the frac tank battery containment liner below the site perimeter earthen berm. The liner extends to the top of the berm. There were some tears observed in the liner, but mainly on the liner on the top of the berm, not near or in the standing water.
2513475 Primary Facility 2016-08-24 Routine/Partial Inspection No Violations Noted
2718984 Primary Facility 2018-04-17 Administrative/File Review No Violations Noted Received & reviewed well site restoration report. No violations were noted. If you have any questions, please contact me.
