0678600437 2016-03-21 Production Wells No Violations Upon inspection of the tank battery I found 2 100 barrel tanks, knockout,separator and meter with no chart. There was leaking pipes, The leadline going into the knockout was leaking a hole had been dug under the elbow going up to make a place to dip oil that was leaking out. The ground was stained with oily substances. There was a line from the bottom 2 plug on the tanks, below the fitting for loading that went over the hill curled around and terminated with an open end 15 over the hill. It would appear as though this was used to drain brine from the tanks. I will wait until Wednesday to determine what actions to take after I have met with Jon Scott. Pictures were taken. The well seemed cappable of production.
0883125179 2016-03-23 Annular Disposal No Violations Accompanied the County Oil and Gas inspector on an inspection of the well and tank battery. The well is equipped with tubing, rods, pumping unit with electric motor and electric service line. The well is connected to a production line flowing to the tank battery. The well had an additional manifold that connected the production tubing to the 4.5 annulus and the 7 annulus. There are valves on each connection to the tubing and casings. There are no check valves installed on the manifold. The well owner was contacted and asked about the manifolds function. The well owner stated that the manifold was used to produce gas from a shallow formation between the 4.5 and 7 casings. The well owner was instructed to install check valves to prevent flow into the annulus. The tank battery had two 100 barrel tanks. Both tank drains were connected to a 2 orange plastic line that extended down slope and terminated at the access road. There were no scald areas or signs of brine contamination near this 2 line. The owner was asked about the function of this 2 line and he stated that it was like that when he took ownership. The owner was instructed to disconnect the 2 lines from the tank drains. RBDMS records show zero brine produced. The well owner was asked how the produced brine was disposed of and why it is not being recorded. The well owner stated that Ohio Oil Gathering hauls the oil and brine. They dispose of the brine. The well owner was advised to record the amount of brine produced. The well owner was told to remove contaminated material from the tank dike. The material is to be disposed at an EPA approved landfill.
0885649646 2016-03-23 Administrative Inspection No Violations I met with Jon Scott to inspect the well. I showed him the mess at the tanks and the line coming from the bottom of the tanks. We then went to look at the strange AD hook up at the well. Itwas determined that all though extremely suspicious especially with no water being claimed in production records that nothing could be proved. The course of action decided on was to contact the owner of the well and ask. I contacted Mike Warner at 12:11 pm. He said he does not use the annular disposal and that the hook up was used to produce gas from the 7 annulus. I insrtucted Mike that if that is the case the connection would need disconnected from the tubing connection and a check valve would need to be installed. I further instructed him the the 2 lines would need to be completely removed from the bottom of the tank and that he would need to submit the brine produced in the production records. The mess at the tank battery will also need to be cleaned up and contamination will need to be bagged up and put into the dumpster with the contamination from the other spill.
