The well has been drilled and stimulated. There is 6 gas processing units on place on location to support each of the 6 wells present in which at this current time all 6 have been drilled but only 3 completed and in production with a total of 6 -210 barrel capacity production fluid tanks present in a plastic lined metal containment dike. In conversation with the newly acquired owners Gulfport Energy they intend on making changes to the well pad perimeter containment by adding functional drain valves at the discharge points along with rebuilding the well pad earthen containment berm. Preliminary Restoration passed on this day.
0122998257 2013-12-03 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the production borehole total drilling depth has been reached at 17,095 and the drilling company finishing up conditioning the well bore and pulling out of the hole with the drill string in preparation to run the production casing. A copy of the casing tally was received.

0851701315 2013-11-20 Production Wells Violations Noted On site electrical inspection with Brad Sellers the Divisions inter agency coordinator and a inspector with the Ohio Department of Commerce- electrical inspector, On the day of inspection deficiencies were found in the grounding system in the internal grounding system inside the electrical panels servicing the site trailers according to the National Electric Code. In conversation with Josh Hinton on the morning of 11/21 He indicated that He would have His contractors fix the problems with their housing.
1283849690 2014-05-28 Completion Testing No Violations Compliance notice follow up inspection, I visited the site along with Division Inspector Joe Hoerst and observed the containment issues noted in my last inspection have been addressed and the company had 3rd party contractors on site working of the final post drilling and stimulation clean up.
1371811978 2013-06-21 Casing String Witnessed No Violations On this day I witnessed the cementing of 1093 of 13.625 surface casing. The mix water was checked and had zero chlorides a PH level of 6 along with a hardness level of 250 ppm. The casing was pressure tested to 400 psi over lift pressure upon the landing of the plug. We had 50 barrels of cement returns to surface.
1384905357 2013-11-19 Casing String Witnessed No Violations Status check inspection on the running of the 9.625intermediate casing, the total depth drilled for this well bore section was 8394 with 8359 of 43.5# P-110 casing ran on the bottom of the string to the point of 3999, at that point is where the 2-stage cement tool is also set at, 40# N-80 casing was used from the DV tool to surface, Baker Hughes cement services were on location to cement the casing in place. The rigs Kelly Bushing is 20.
1400169315 2014-05-15 Completion Testing No Violations Status check inspection on the well completion process found Stone Energys work over rig on the hole running the production tubing in the well bore and at the time of my arrival the well head fluid catch can used to recover well bore fluid was not upright and fluid was leaking onto the ground surface and no fluid was being recovered from the tin horn, also at various places on the site the working area containment boundaries was damaged allowing fluid to leak off site into the storm drain system that did not have proper containment being kept up around them also, the 500 barrel flow back tanks were found not to have grounding in place and had improper containment erected, the site in general was found to have debris and trash scattered about improperly discarded from the contractors. Instructions were left with Glenn Boyd on this day are as follows- Maintain proper fluid recover during work over operations and recover fluids from the tin horn, maintain proper containment around the site and fluid storage areas, place boomers and spill rags in areas needed to recover and fluids generated from the site, bring in a water truck and recover the surface fluid from the site that has been in contact with well related materials, remove debris scattered through out the site. 1441101927 2017-06-27 Production Wells No Violations Status check inspection found 6 wells present on location and in production. All wells are equipped with tubing heads and the production valves are in the open position. A total of 6 gas processing units are present on location. A total of 6 production tanks are present inside a polymer lined steel containment dike area. Proper signage is posted at the well pad entrance on the day of my inspection.
1712076144 2015-02-26 Preliminary Restoration No Violations Preliminary Restoration status check inspection found a production tree present at the well head with the production valve in the open position with a flow line leading into a gas processing unit. There is 6-200 barrel capacity production tanks located inside a mechanical dike area. Preliminary Restoration passed on this day.
1768699948 2013-11-24 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 9,474 and the current borehole position nearing the bottom of the horizontal curve section with a projected well bore total depth to be drilled of 17,000 using a 8.5 Auto Track Directional drilling tool with the synthetic oil based drilling mud weight of 13.5#.
