I noted that the signage had become faded and notified Ron Gibson about the need for maintenance. I walked and inspected the areas surrounding the well and battery and found normal greenscape with no signs of problems or impacts. I had noted during a previous inspection that one of the stock tanks was severely rusted, with several holes. I had advised Ron Gibson that the tank must not be produced into to, and must be removed. He had advised that production into the tank had stopped, and removal of the tank was pending. The battery is shared with the #2-0914 well, located on the same parcel. |

The areas surrounding the wellhead were comprised of an open agricultural field within a larger commercial nursery facility. I walked the area surrounding the wellhead and did not observe any signs of impact or problems. The associated tank battery is located near the main roadway and is shared with the #-2-914 unit. The load-lines were plugged and secure. The tank lids were closed. The secondary containment was dry and free from signs of leakage, spills or other problems. The access road to the battery and well is a shared-use road with the nursery facility and was in good condition. There were no issues or problems requiring attention at the time of this inspection. I noted that there was no apparent production report filed for the 2015 reporting period. I contacted the well owner and he indicated that he would investigate to determine where the missing information was. As of 11/04/2016, the owner had electronically transmitted the Form 10 annual production data for 2015 to ODNR in Columbus. |

The 140 BBL tank has been emptied, cleaned out and disconnected from the other production equipment, but was still inside the diked battery. Signage was located at the well and tank battery. The tank battery was also fenced with a locked man door and shared with well 2-0914. I contacted Ron Gibson of Ronald A. Gibson & Assoc Inc. regarding the removal of the 140 BBL tank. |
